Cremation Secured Forms

The third step is to complete the legal authorizations that allow us to perform the cremation.  In Texas the legal next-of-kin is/are the only person(s) that may authorize the cremation of a deceased person (unless prior legal arrangements have been made).  Texas Law requires that in addition to the Cremation Authorization, you sign a Denial of Embalming, a Disclosure and Disclaimer and a Statement of Goods and Services.

On this page, you will need to click the link so that you can securely sign the necessary forms.

When you click the link, it will take you to a new page.
